Other Philippine-government issued identification cards such as Land Transportation Office issued driver's license, social security cards like the Government Service Insurance System (CSIS), Social Security System (SSS), Professional Regulatory Commission (PRC), philippine postal ID, Commission on Elections (COMELEC) Voter's ID, etc., showing year of birth as reference point for the centenarian's eligibility for the award and incentive.

Any two (2) of the following secondary documents may also be accepted in the absence of the abovementioned primary IDs:

Marriage Certificate of the centenarian issued by the philippine Statistics Authority (PSA) or the Local Civil Registrar (LCR)

Birth Certificates of children born by/of the centenarian issued by PSA or LCR

Affidavit executed by at least two (2) disinterested persons should be at least aged 80 years old and above with personal knowledge of centenarians actual age or date of birth

Old school or employment record showing date of birth of centenarian

Baptismal and/or confirmation records of centenarian certified by the parish Church and other religious denominations

Medical and/or dental examination issued by government/private doctors or dentists

Other related documents, i.e., Certificate from National Commission on Muslim Filipinos (NCMF) National Commission on Indigenous people (NCIP), ATPSLAI, AMWSLEI, and PVAO.

2. List of beneficiaries should be endorsed by the Municipal/City Social Welfare Officer together with the supporting documents at the Provincial Social Welfare and Development Office-Lingayen.

SECTION 6. ROLE OF THE OFFICE OF THE PROVINCIAL SOCIAL WELFARE AND DEVELOPMENT - To carry the goal of this Program, the PSWDO shall be responsible for the thorough review and evaluation of centenarians to be submitted to the Provincial Governor for his final approval.

The PSWDO shall determine the manner by which the centenarian shall receive the award and cash incentive. The PSWDO may be assisted by the City/Municipal Social Welfare and Development Office.

SECTION 7. APPROPRIATIONS - The projects and activities of elderly shall be allocated under the Special Purpose Appropriation.
